One of the better Sight systems I have worked with was on a Norsman Motor Yacht.
Heavy push buttons open the tank valves to the sight tube, then they close automatically when released.
    The glass tubes were protected.
One of the worst systems was a Super Nova 80 that used ball valves and clear plastic hose.
And I could not reach the bottom valves on some of the outboard tanks with the 3412s in the way.

On DeFevers and other lil boats, I tried to explain to owners to keep those valves closed till really taking a read and closing them when complete. Unbelievable attitudes (bad) I'd get back.

Years ago on a delivery I got a bilge pump briefly cycling on once. Twice. Went down to the ER and found diesel in the bilge. Quickly found the source, it was pouring in from the Stbd fuel tank sight tube which had come loose. Turned it the tube was partially ripped out and valve pulled open by the tech who had just installed a watermaker when he had to squeeze between the engine and the tank...

    Lesson learned

As to this report, amazing how the USCG inspected the vessel and passed it. They mandate a bunch of stupid placards but that s one that should be added “sight gauge valve must be kept in closed position”
