2014-built Admiral superyacht "Ethos" (the ex. "Entourage" ) was taking on water in Greece. It has been identified that crew left a door opened...likely the transverse tender garage door on the starboard side.
Crazy that this could happen as its virtually an impossibility to get underway with shell doors open and not know unless safety systems were compromised as all class vessels are fitted with alarms / proximity switches and sensors on shell doors or transom doors so if this happened these systems were most likely bypassed or disabled. Most vessels engines won't start with open shell doors unless the alarms are acknowledged and over-ridden from the bridge and ECR.
remember the one that sank on the New River in FTL a few years back because of an open transom door and the wake of a couple of tugs? Note to self. Time to test the hydraulic and 240v emergency pumps again
Are there indicator lights or alarms at the helm that alert the captain/crew of critical doors left open ? Seems like a dumb question given the circumstances but if not, why ?

Accidents happen, A 70,000 ton cruise ship sank in a German ship yard in 2007 because a door was left open near the water line and a storm came up
Ethos is Maltese flagged & registered at 499 tonnes; which incidentally is one tonne under the compulsory minimum crew manning requirements. A yacht of this length, beam & tonnage operating with 5 crew is certainly questionable .
